Our Specialized Community Services (SCS) team can facilitate the coordination of healthcare after a person is discharged from a hospital or healthcare facility. This care incorporates different types of therapy for healing and can be administered in your home, nursing homes, private care homes, long-term care facilities, or at one of our specialized community homes. SCS can also support short term placement for children and adults when there is an interruption to their permanent home and provide housing, support and other services are required.
